Whilst using the website or survey products, you may be asked to provide certain information by which you can be identified. We only collect your personal information where necessary for one or more of our functions or activities. This information may include, but is not limited to the following: Name, age, country of residence, and other information relevant to the survey. Survey “Administrators” provide their email address for correspondence purposes.
Your responses to surveys on this website are entirely voluntary. They are used to generate a report, based on the response that you provide. The data may be used anonymously, in ongoing research. Your item scores and responses will only be in summarised in group data. The “Administrator” of the surveys, will be the person that will see your individual summary data, and it is at their and your own discretion who else sees your summary data. The sport well-being survey is suitable for approximately 11 years and older. Those under the age of 18 must have an approval from an adult “Administrator” of the surveys to gain access to the website and complete a survey. If the “Administrator” deems it appropriate, they may choose to have a parent, guardian, teacher, coach to also authorise access to and use of the surveys.
Some identification will need to be used to help the group administrator differentiate between users, and this can be name or anonymous number. We do not ask for user or children’s address, e-mail address, or any other information that could be used to identify a user or child, except through you.
